A fear ladder is a list of situations you avoid, ranked from least to most distressing, so you can face them one rung at a time. This is the general ladder for everyday anxiety and phobias. It is the core of graded exposure, the best-supported approach for anxiety and phobias. Each rung gets a SUDs rating (Subjective Units of Distress: 0 is calm, 100 is the most anxious you can imagine), which sets the order and the pace.
Start on a moderate rung, around 40 to 50, and stay with it until your anxiety settles or you learn the feared outcome did not happen. Repeat a rung until it feels routine, then climb. For a dog phobia, the rungs might run from looking at photos (SUDs 25), to watching a leashed dog across a park (45), to standing near a calm, held dog (65). This is collaborative and paced with your therapist, and you are never pushed up a rung before you are ready. - List the situations, places, or activities you avoid. Write each as a specific, concrete action, not a vague goal.
- Rate each one's SUDs from 0 (calm) to 100 (most anxious imaginable).
- Order them low to high, aiming for roughly 10 to 20 points between rungs.
- With your therapist, start on a rung around 40 to 50. Stay with it until the anxiety eases or the feared outcome does not happen.
- Mark the "Practiced?" column each time, and repeat a rung until it feels manageable before climbing.
- Update the ladder as scores change: add rungs, split big steps, and re-rate as your confidence grows.
