How to use this worksheet

  1. Describe the situation that triggered a strong emotion.
  2. Name the emotion and rate its intensity (0-100%).
  3. Write the automatic thought — exactly what went through your mind.
  4. List evidence for and against the thought.
  5. Write a balanced thought based on the evidence.
  6. Re-rate the emotion in the outcome box.
